The National Examinations Council of Tanzania (NECTA) processes requests to correct candidates’ names (Kubadilisha Jina Cheti Cha Form Four) that were misspelled in the Primary, Secondary and Teaching examinations. The procedure for submitting requests and making modifications is detailed below:

  4. Examination Name: The only basis for name correction is the name recorded in the examination records. Names from court affidavits, birth certificates, marriage certificates, baptism records, etc., are not considered for this amendment.

  1. Applications handled by the school, the Principal/Head Teacher should fill out the form correction or ‘Query Circular Form’ specifying the mistake made in the name and title the signature should be written.
  2. Applications handled by the parent/candidate; fill out the name correction form available on the website of the Examinations Council.
  3. Pay the name adjustment fee of sh. 35,000/= (for one certificate) through banks NMB, CRDB and NBC or mobile networks (M-Pesa and Tigo Pesa) using ‘Control number’ to be issued by NECTA.
  4. Submit the NECTA application form attaching the wrong certificate/result document name and payment receipt.

After the correction is done, the Results Slip or a new certificate with the correction will be sent to the principal or to the candidate (if not a student).
